Case Manager


Department: Treatment Care and Support

Location: South East Regional Health Authority

Three (3) positions available

The case manager will offer a range of client-centered services that link PLHIV with health care, psychosocial, and other services. This will include the  coordination and follow-up of medical treatments, timely and coordinated access to medically appropriate levels of health and supportive services and continuity of care, through ongoing assessment of the client’s and other key family members’ needs and personal support systems. The case manager will play a key role in tracking outcomes and increasing the number of patients achieving viral suppression. The objectives of HIV/AIDS case management for persons living with HIV/AIDS are:

  • To ensure early access to health care and social services
  • To improve integration of services
  • To improve linkage to community services
  • To improve uptake of ART
  • To improve continuity of care
  • To increase retention rates among PLHIV on treatment
  • To improve compliance with medical and service specifications
  • To reduce risk behaviors
  • To facilitate personal empowerment
  • To improve quality of life
  • To improve health outcomes

Appropriate categories of staff in the Jamaican health and community (NGO) systems from which case managers may be selected include:

  • Contact Investigators
  • Social Workers
  • Treatment and Care Coordinators
  • Nurses

 

Required Competencies

  • Proficiency in the use of relevant computer applications (MS Word, Excel and Power Point)
  • Communication skills – Oral and Written, along with group facilitation and counselling skills, programme development and proposal writing.
  • Able to work on own initiative with minimum supervision.
  • Flexibility (including some Saturdays, Sundays and Public Holidays) and adaptability.
  • Ability to develop, plan, and implement short and long-range goals.
  • Ability to coordinate and implement multiple projects and events.
  • Ability to handle high volumes of activity in a timely, professional and efficient manner.
  • Skill in organizing resources and establishing priorities.
  • Ability to use independent judgment and to manage and impart confidential information.
  • Ability to work professionally and cooperate with team members and representatives of other agencies
  • ability to be a team player
  • Strong time management skills, especially when facing tight deadline

 

Primary Functions

  • Assessment: obtaining information from the clients about their needs resources and wishes.
  • Planning: mutually agreeing on goals for participating in case management, planning activities to address identified needs and developing strategies to achieve goals.
  • Linkage: actively connecting clients to needed services and resources
  • Monitoring: methodically assessing clients’ progress towards the goals and objectives that are stated in their service plans.
  • Advocacy: ensuring that services are delivered in a timely manner and provides support to address barriers to care that occur along the continuum

 

Scope of Work

Once a client is identified as suitable for case management services, the client should be referred to the case manager who will then initiate the following key activities:

  • Assessment of service needs
  • Development of an individualized service plan;
  • Coordinating services and activities required to implement the service plan;

– Refer to appropriate agencies required to assist the client in achieving the goals and objectives identified in their service plan and provide client-specific advocacy

  • Monitoring to assess the efficacy of the service plan;

– Participate in case conferencing with other members of the case management team

– Evaluate service plan goals and conduct follow-up

– Re-evaluate and adapt the plan as necessary

-The following clients should be reassessed within 60 days.

– Incarcerated within the last 90 days

– Diagnosed with HIV in the last 180 days

– Currently homeless

  • Transfer and inactivate clients as appropriate

Other specific deliverables

  • Provide one-on-one, family and group counselling in the areas of managing personal hygiene and nutrition, identifying and taking immediate informed action to manage grief, death and dying and medical needs to PLWHA, their family members, peers, friends, etc.
  • Coach, counsel and advise family members of PLHIVA about needs, care, support and treatment.
  • Makes regular visits to clients when hospitalized.

 Special service plans must be developed for clients who fall into key populations.

These activities will be guided by the Case Management Protocol.

 

Note

Reports directly to the Medical Officer (Health) with HIV Responsibility and the Regional HIV/STI/Tb Coordinator.

Salary commensurate with candidate’s experience and qualifications.
